Planning for the future often means thinking about finances, retirement or travel. But one of the most meaningful plans you can make is for your health care and other important decisions near the end of life.

Jefferson Healthcare is inviting community members to take an important step toward making these plans with a free end-of-life planning seminar on May 19 at 1 p.m. at the Port Ludlow Bay Club.

Free advice from Jefferson Healthcare experts

This session is designed to help individuals and families better understand their options, document their wishes and approach these often-difficult conversations with greater confidence and clarity.

The seminar will be led by Dr. Joseph Mattern and Jill Newsome RN, BSN of Jefferson Healthcare Home Health Hospice. Together, they will guide attendees through the essentials of advance care planning in a way that is practical, approachable and grounded in real-life situations.

What to expect

Early preparation can reduce stress for loved ones and ensure that personal wishes are honored. In this seminar, attendees will learn the basics of advance care planning, including the purpose and limitations of key documents such as advance directives, living wills and durable power of attorney for healthcare.

The seminar will also introduce the “Five Wishes” framework, which helps individuals articulate not only medical preferences, but also personal, emotional and spiritual needs.

In addition, participants will gain a clearer understanding of care options, including the differences between curative treatment, palliative care and hospice, as well as where care can take place.

Starting the conversation

This session will also offer guidance on how to start meaningful conversations with family members and how to choose a trusted healthcare decision-maker.

Attendees will also learn what to expect from medical teams when patients are unable to speak for themselves, and how decisions about care are made in those moments.
